2010 Registration Overview 2010 Registration And Code Of Conduct Form 2010 Medical Form 2010 Parent Participation Form 2010 CORI Form Refund Request Form 2010 PAPERWORK CHECKLIST: • Registration Form - Please leave the section that says Grade Point Average blank. • MEDICAL RELEASE & TREATMENT FORM - must be dated after January 1st 2010 on form PROVIDED. Alternate forms will not be accepted. The doctor’s name, address, and phone number must be stamped or printed on the form. • Original Birth Certificate - All new Players and Cheerleaders must provide original birth certificates with a raised seal (they will be returned at some point during the season). • Copy of Final Report Card (2010) – Name of school and grade as well as all pages must be provided. (You may white out the comments) • Parent Code of Conduct Form - Signed and dated. • Parent Participation Form – Filled out complete unless the opt out option has been chosen. • CORI Form - if you plan on volunteering in any capacity, please fill out the form and include copy of license. For your convenience there will be a final collection day scheduled sometime in June. Please refer to the website for the exact date. The responsibility of compliance is yours, not NMM Pop Warner Participants will not practice or be issued equipment until all forms are completed. – NO EXCEPTIONS. |
