2011 PAPERWORK CHECKLIST:

• Player Contract Form – Signed by player and Parent

• Medical Release Forms – Section I. Parent/Guardian completes and signs & Section II. Physician completes and signs.

• Original Birth Certificate - All new Players and Cheerleaders must provide original birth certificates with a raised seal (they will be returned at some point during the season).

• Copy of Final Report Card (2011) – Name of school and grade as well as all pages must be provided. (You may white out the comments)

• Check/Cash/Money Order - For registration fee for those not paying by CC/Debit.

• Parent Code of Conduct Form -Signed and dated.

• Parent Participation Form – Filled out complete unless the opt out option has been chosen.

• CORI Form - if you plan on volunteering in any capacity, please fill out the form and include copy of license.

For your convenience there will be a final collection day scheduled sometime in June. Please refer to the website for the exact date.

The responsibility of compliance is yours, not NMM Pop Warner

Participants will not practice or be issued equipment until all forms are
completed. – NO EXCEPTIONS.
