Paperwork Checklist:
Player Contract Form – Signed by player and Parent
Medical Release Forms – Section I. Parent/Guardian completes and signs. Section II. Physician completes and signs.
Original Birth Certificate - All new Players and Cheerleaders must provide original birth certificates with a raised seal (they will be returned at some point during the season.)
Copy of Final Report Card (2013) – Name of school and grade as well as all pages must be provided. (You may white out the comments.)

Having just completed our April Meeting this evening we want to pass on a few notes and requests regarding our upcoming season. At the present time we are in the midst of registration, recruiting coaches and finalizing equipment hand out logistics.
Our first priority is getting our players registered by April 19th. PLEASE REGISTER YOUR SON OR DAUGHTER BY APRIL 19TH TO AVOID A $25.00 LATE PROCESSING FEE. In order to finalize our teams , coaches and game schedules we must have all teams selected no later than May 1st. We can not stress how important this is to ensure your son and daughter have a team to play on this fall.

The NMM Board of Directors would like to take this opportunity to invite you to help out a fellow VIKING. As many of you know, Taylor Sack suffered a horrific ski accident back in February that left him paralyzed. Taylor Sack played for the Vikings for 4 years and by all accounts, after speaking with his former coaches, is a role model for all of us to follow. In an effort to support Taylor and his family our board has voted to donate $1,000 dollars to help support Taylor and his families support costs.
A family trust has been set up at:

The Medfield branch is located at 495 Main Street.
